I've got a couple of extra Mac versions of the Voodoo 5 (PCI), and was
wondering if it is possible to flash one for use on a PC. If it is
possible, where should I go for the ROM image, utilities, and drivers?

Yes, it's certainly possible. I've done it myself and it works quite nicely.
Here's a page for BIOS files and flashing utilities:
http://homepage.ntlworld.com/g.stanton/3dfxbios/. Remember that if you want
to see what you're doing, you have to have a non-3dfx PC video card in your
computer aswell while you flash the VoodooMAC.

should have everything you need. Also, this site:

No idea, never tried to. You *can* flash a Mac version with the PC BIOS,
although that disables the DVI-out (IIRC there might be a modded BIOS out
somewhere that allows DVI on PC), but I don't know if it's possible the
other way too.
